Chronic Illness Care Management

Chronic Illness Care Managers, employed by the MMC PHO, are available to patients identified as having difficulty reaching treatment goals. Working with the physician as a member of the practice team, these nurse care managers coordinate patient care by arranging access to healthcare services, providing patient education, and promoting self-management.
