Practice Redesign
The MMC PHO Practice Redesign Program has a team of Practice Improvement Advisors who have years of experience and training in healthcare process improvement. Our team members are trained Dartmouth Clinical Microsystems Coaches and are certified TeamSTEPPS Master Trainers.
Through education and coaching, the team works closely with providers, mid-levels and interdisciplinary teams to provide them with the tools, skills and knowledge they need to continually improve the care they deliver.
